Support Coordinator

Key Responsibilities:

  • Assist NDIS participants in understanding and implementing their NDIS plans.
  • Connect participants with appropriate service providers and supports.
  • Monitor and review participant goals and progress.
  • Maintain accurate records and reports.

Requirements:

  • Experience in NDIS support coordination or case management.
  • Strong understanding of the NDIS framework and funding categories.
  • Excellent problem-solving and communication skills.
  • Ability to work independently and manage multiple clients.
